Equity and patient pathways monitoring in SW Moravia

The pilot model of CCCN is based on a fully standardised multilevel information system which covers all dimensions of cancer care and is uniformly implemented in all members of the CCCN. The system incorporates a fully representative population-based data collection (100% coverage), hospital information systems, as well as data gathered from associated cancer care facilities and follow-up units (screening centres, primary care, palliative care etc.).

The presented data view is the output of a pilot analysis of all levels of the implemented information system, carried out with a special focus on patients’ migration from or to the CCCN region. The analysis also documents that both population size and cancer burden in the catchment area of the CCCN are high enough in order to design and to perform clinical trials, relevant “real-world” observations or surveys, and to generalise expertise in cancer management for the whole country. The pilot model of CCCN also facilitated the redistribution of care within the network. Care for most common cancers is organised locally in all access points, while services for rare cancers, haematological malignancies and childhood cancers are centralised within the CCCN. The CCCN also works as a centralised treatment point for neighbouring regions.
